Health Extension for Diabetes

Health Extension for Diabetes is a free diabetes self-management education and support program recognized as a practiced-tested support program by the American Diabetes Association. This program's primary goal is to help you better manage your blood sugar to prevent or delay complications.

 

The program includes a series of eight education sessions and support sessions over four months designed to help you learn more about diabetes and provide you with the necessary skills and support that you need to successfully manage your diabetes.
