Do you need help better managing your diabetes? Health Extension for Diabetes is for people who have been diagnosed
with either type 1 or type 2 diabetes.

The program provides participants with:
• diabetes education;
• self management resources;
• support to better manage diabetes.

Group members meet for 1 hour every other week for 8 education sessions with intermittent support groups to share successes, challenges, and learn about diabetes.

During the program, group members also receive ongoing support and health education through weekly health coaching calls.
